Follow these simple steps
- Determine whether you have a qualifying condition under Oregon law.
- Fill out our registration packet.If you have never been to Compassion Center, fill out the New Patient Registration. On the Authorization for Release of Medical Records, authorize the release of records from January 1, 2012 to the present date.If you are a returning patient, fill out the Renewal Registration. On the Authorization for Release of Medical Records, authorize the release of records from January 1, 2013 to the present date.if you need records released from the Veterans
Administration, fill out this medical release form.*You can email the registration packet to email@example.com. If you are emailing us your
registration packet, do not include your credit or debit card information – just give us a call and we will complete the transaction over the phone.You can also fax it to 541-484-0891, mail it to 2055 W 12th Ave, Eugene OR 97402, or bring it by our office during working hours.
- Turn in your registration to Compassion Center and wait for a call from us.Once we receive and review your medical records, we will call you to schedule an appointment.
